How to download a visit summary as a PDF

Two routes in the patient portal — from the appointment card and from your health records. What is in the file, what is not, and what to ask for when you need more.

After a visit the practitioner writes a summary — what you did, what you should do at home and when to come back. You can read it in the portal, but more often a file is what you need: to print, to show another doctor, for your own records. This guide walks both routes.

How to do it

Sign in to the portal

Go into the patient portal with the same account you accepted the clinic's invitation on. You land on the Today screen.

Route one: from the appointment card

Click Appointments in the top menu and open a visit that has already taken place. If the practitioner has written a summary, a View Summary button appears on the card — and under the expanded text, three things:

  • Download summary — builds a PDF and saves it to your disk.
  • Print / Save as PDF — opens the browser's ordinary print dialog. Useful when you want to print straight away.
  • View full summary in Health Records — takes you to the tab described in the next step.

Route two: from your health records

The Health Records page gathers everything in one place, in four tabs: Summaries, Notes, Programs and Assessments. In the Summaries tab every entry has its own Download summary button.

This is the easier route when you want several summaries at once, or you cannot remember the date of the visit.

Check what is in the file

The file is built from whatever the practitioner filled in after the visit. Empty sections simply do not appear. You will usually find:

  • the diagnosis or clinical impression,
  • the treatment performed,
  • the home exercise programme,
  • the lifestyle recommendations,
  • referrals and the follow-up plan.

At the top there is also an Important Medical Information box — allergies, current medication and chronic conditions — and, if pain locations were marked during the visit, a body map. The footer carries a sentence about the informational nature of the document and the name of the person who prepared it.

Know which language it will come out in

The summary is built in your language, if the clinic has recorded one on your patient record — regardless of the language the practitioner works in. Where the record has no language on file, the document comes out in the panel language of the person generating it.

Changing the language after downloading does not rebuild the finished file. Ask the clinic to correct the record and download the summary again.

What this file does not contain

A visit summary is a document for you, not the full medical record. You will not find the whole clinical note in it, or the complete set of measurements.

You get the full records and a copy of all the data the clinic holds about you by another route: in the menu under your avatar open Privacy & Consent and use Download My Data. That is a request under Article 15 GDPR — the clinic has a statutory deadline for it. Details in data subject rights.

When something does not work

What this means: The practitioner has not written the summary yet, or the visit has not been closed.

What to do: Give it until the end of the working day. If there is still nothing after two days, ask at reception.
Error

Could not generate PDF. Please try again.

What this means: Building the document was interrupted — most often on a weak connection.

What to do: Wait a moment and click again. If it recurs on the same summary, use **Print / Save as PDF**.

What this means: The browser blocked the print window.

What to do: Allow this site to open windows in your browser settings and try again.
